To request driving test appointment please complete the info request form on the right hand side of this page. You can do this on a desktop computer, your smart phone or below on a handheld device. Please remember to include your examination permit number and a validation number. The examination permit number is circled in red on the picture below and the validation number is circled in yellow to help you locate them.
